The adoptee agrees to make a NON-REFUNDABLE donation of $_________________ to be eligible to adopt this animal.
Once the animal has been adopted and leaves the custody of Country Oaks Veterinary Hospital, all care and expenses including veterinary care, is the responsibility of the ADOPTEE. There is no discount for future emergency services and treatment given to the owners of animals adopted from Country Oaks Veterinary Hospital.
The Adoptee agrees to provide care including but not limited to heartworm prevention, flea control, proper food, vaccinations, dental care, and medications. The Adoptee agrees to keep the animal confined and not allow it to roam off leash.
The animal has been examined by a licensed veterinarian and has been found to have no medical problems except any listed above in the animal description. There is no guarantee that the animal is completely healthy. There is no guarantee that the animal does not have any other diseases or problems not detected upon examination. It is possible for an animal to carry a disease that can make other animals or people sick. If any health problems are noted after adoption, consult your veterinarian for evaluation and treatment. The Adoptee agrees that Country Oaks Veterinary Hospital and staff are not responsible for any future health problems of this animal or any people or other animals.
The behavior of the animal has been normal, but there is
no guarantee that the animal will not become aggressive or otherwise display
undesirable behavior that will make it unsuitable to be a pet.
